Someone pointed this on the IMDb, and they are ABSOLUTELY correct: Kavanaugh posts a picture of Terry Crowley on his board during his investigation. It is the picture with the bullet hole in his face and blood pooled around his head.
However, if you'll remember, Terry wasn't found DOA; he died a few hours later in the hospital, so that picture is a goof. You could argue that the picture was taken before they rushed him to the hospital, but if the guy's still alive, that would likely NOT happen as they would be doing everything in their power to rush him to the emergency room, not snapping a photo for posterity.

I remember someone bringing this up a while back (though on a different board).
The Shield has never had much luck with photos, in my opinion. I remember the end of and episode in S3 where Aceveda is beginning his vendetta against Juan, looking at Juan's picture in a photo array book. The picture was clearly a picture of someone else with Juan's face haphazardly photoshopped on top of someone elses. The biggest giveaway was that the photo said he was 5'7. They must have realised how bad it looked because a few episodes later they changed it to an actual photo of Juan.
This is only mildly distracting compared to the Terry thing, though as having a photo of Terry on the floor of Two-Times' apartment is kind of a plothole.
These are only minor issues though. I was glad when I read this thread that it was about something I was already aware of. The Shield is one of the few long-running TV shows I've followed that doesn't have any major logical inconsistencies in its storylines as a sresult having gone on for a long time.

It's not a plot hole in the sense that it undermines alot of key elements; it's just a prop goof, if anything.

Eh... I think it's somewhere between a plothole and a prop goof. Someone should have known better, imho. Having Terry's picture up on Kav's board was an important aspect of Kav's method.... it made everyone uncomfortable. Terry's morgue /autopsy shot or his offiial portrait in uniform with a big smile would have had a similar impact AND been within reason. This plot goof didn't ruin the show for me, though.

Of course, they could have taken his dead body back to the crime scene to re-enact the shooting for the photos for the case.

it didnt ruin it for you im sure it didnt ruin it for the rest of us,
hell terry coulda been smilin and winkin at the cam and had the bullethole in the face, and i still woulda been like damn kavanaugh is crazy m*therf*cker just wont let up, jajaja

What if the photo Kav had of Terry was taken after he was dead and laying on the gurny in the hospital? I realize it's unlikely, i think i even remember the same red carpet background like in the freeze frame from Pilot, but hey, I'm trying to defend our show here
